The role of occupational medicine at modern enterprises is naturally shifting from formal medical examinations to proactive employee health management. High morbidity rates, which resulted in the loss of over 1,500 working days per 100 employees, forced enterprise management to seek new approaches. The problem was further aggravated by a lack of a systematic approach: health screenings, periodic medical examinations, and preventive care operated in isolation, while fragmented medical data prevented effective management. In his presentation, Ilya Lubnin shares the experience of a comprehensive health preservation system transformation that significantly reduced morbidity and built effective collaboration between production and medical teams.

Routine Optimization: How to Reduce Time Losses on Medical Examinations

Traditional medical examinations often become an area of conflict between production targets and the need for health checks: the process could drag on for an entire workday, creating chaotic queues and causing discontent among line managers. To resolve this issue, a strict routing system was implemented.

Instead of visiting doctors independently, each employee receives an individual routing slip in advance, specifying the exact time and room where the examination begins. A continuous flow system built on the principles of production logistics eliminates redundant movements and waiting time. In parallel with medical exams, health screening flows have been effectively structured. As a result, the time required to complete visits to all specialists was reduced to two hours, allowing shop floor managers to release staff without compromising production targets.

An additional optimization tool was health checklists completed by employees in advance. The questionnaire, containing up to 70 questions, collects a detailed medical history regarding lifestyle, unhealthy habits, and hereditary diseases. The structure of the checklist is visually divided into normal and risk columns. This allows the general practitioner to skip basic questioning during the appointment and immediately focus on problem areas, providing targeted recommendations.

Health Monitoring and Prevention at the Shop Floor Level

Health management is impossible without understanding the objective local picture and ensuring leadership engagement. To achieve this, the enterprise introduced department "health passports" reflecting morbidity dynamics compared to previous periods. These indicators are regularly discussed at shop-level safety meetings with the participation of a general practitioner assigned to the shop floor.

A noteworthy practice was the joint analysis of off-the-job injury cases. Statistics showed that injuries sustained outside of work contribute significantly to the total number of sick leave days. Causes of such incidents are analyzed at meetings without unnecessary personalization. The logic is simple: if an employee possesses a high level of safety culture at work, this skill should extend into daily life, eliminating unsafe behaviors at home.

Grassroots initiatives are actively used to promote a healthy lifestyle. As part of the internal "Safety Territory" contest, points are awarded for independent health improvement activities conducted by departments: from hosting workplace exercise masterclasses to smoking cessation campaigns, where the shop head publicly rewards employees who quit smoking. In parallel, volunteers work at gate checkpoints offering to swap cigarettes for sweets.

Integrating Medical Services into the Production Environment

To bridge the gap between physicians and production personnel, a series of familiarization tours was organized for shop general practitioners directly at the workplaces. By observing actual working conditions and assessing the impact of harmful factors firsthand, medical professionals gained a deeper understanding of job specifics, which enhanced the objectivity of medical decisions.

In addition, "Health Schools" operate at the enterprise: specialized medical experts visit production shops to deliver lectures on preventing chronic diseases and unhealthy habits. The employer also assumes targeted funding for the early detection of critical health issues, initiating additional cancer screenings and inpatient hearing loss treatment programs.

A systematic approach was also applied to vaccination. Instead of coercion, the emphasis was placed on demonstrating to shop managers a direct correlation between the vaccination rate and the morbidity level in their units. Understanding this link, department heads motivate their teams independently, while the walk-in availability of occupational health posts makes the vaccination process quick and convenient.
